Job: Discharge Advocate - RN Case Manager(Summerlin)

Jobing Description

As the Discharge Advocate (RN Case Manager) you will be involved in this new program that we are implementing. You will be onsite as Summerlin hospital working Monday - Friday 8AM - 5PM. This role focuses solely on the discharge process of our members at the hospital.

Responsibilities for this position include:




  • Reviews daily census



  • Reviews medical records for all new admissions



  • Daily interaction with patient to teach and reinforce treatment plan information and to identify barriers/gaps in discharge plan



  • Works with patient and family members on needs and requests



  • Schedules follow-up appointments for patient



  • Educates patient and family members on illness and medications. Reviews medications to be taken at home, discuss any possible home care (either family or home health, etc)



  • Patient education



  • Special projects, initiatives, and other job duties as assigned

Skills / Requirements

Requirements:




  • Current, unrestricted RN licensure in the state of NV



  • 3 or more years of Clinical RN experience, resulting in excellent clinical skills and judgment



  • 1+ years of experience as a case manager



  • 1+ years of experience with discharge planning and chart review




  • CCM within 2 years of employment



  • Proficiency with written and verbal communication skills



  • Basic level of experience with Microsoft Word, with the ability to navigate a Windows environment



  • Intermediate or expert level of proficiency with being able to assess and identify needs and being able to influence and motivate



  • Understanding of change and ability to move members along the continuum of change



  • Expert conflict management skills



  • Able to work under pressure and appropriately prioritize responsibilities



Additional Preferred Assets:



  • Bachelor's degree in Nursing preferred



  • Coaching and/or decision support experience



  • Ability to adapt well to change and take on new challenges/projects
